Earlier that year in a Winlock plant, another employee severed her hand off in a piece of equipment. That summer, another employee in a Chehalis plant also suffered hand injuries from a glass drilling machine. Following these accidents, a review was conducted for a total of eight safety or health inspections at the three Cardinal plants between 2007 and 2008.

While time has improved technology, it has not fixed all of Cardinal Glass’ woes. In 2017, an employee died at a Durant, Okla., float glass plant after becoming stuck in machinery.

Death benefits
